What are insurance premiums?
Premiums: are a prepaid payment or series of payments made to a health insurance plan by purchasers, and often plan members, for medical benefits.
What is a deductible?
Deductible: a set dollar amount you pay each benefit period for covered services before your health insurance policy begins paying benefits. Deductibles are reset each year.
What is copay?
Copay: a fixed dollar amount (for example $20 or $30) you pay for specified covered services such as a doctor office visit. A copay applies each time the service is provided.
What is coinsurance?
Coinsurance: a percentage (for example 20%) of the allowed amount you pay for a healthcare covered service. Coinsurance applies after the deductible has been met.
What is out-of-pocket maximum?
Out-of-Pocket Maximum: a fixed dollar amount that is the most you will pay for deductibles and coinsurance for most covered services in the course of a benefit period.
Once the out-of-pocket maximum is met, most covered services are paid at 100% of the allowed amount.
What is wellness coverage?
Wellness: benefits that pay for routine physical exams and diagnostic lab and x-ray services. For example mammograms, prostate specific antigen (PSA) tests, etc.
What is prescription drug coverage?
Prescription Drug: benefits for prescribed medications that are typically received from a pharmacy. Benefits vary on the type of medication that is purchased: generic vs. brand. Often benefits can be a copay per prescription filled or coinsuranace.
